The gardener Burton and his wife Michelle have just lost their sick child because of the Viscount of Belfor\u2019s refusal to let him be treated. Upon hearing their story, Moriarty provides them a plan to kill the Viscount, one that turns his own heart condition against him rather than endangering the lives of Burton and Michelle. So as long as they played their roles correctly, Burton and Michelle would finally be freed from their grief and remorse.<\/span> <\/span><\/span><\/p>\n<figure class=\"wp-block-image to take down the nobles, this one takes a special place in my heart because it\u2019s medical-related. It is not a coincidence nor a contrived plot point that grapefruit juice is introduced as the crux to \u201ckill\u201d the Viscount. There is actual <a href=\"https:\/\/buradabiliyorum.com\/en\/category\/sciencee\/\" data-internallinksmanager029f6b8e52c=\"5\" title=\"Science\" target=\"_blank\" rel=\"noopener\">science<\/a> behind the complex relationship of quinine and grapefruits that is still being discussed today, so let\u2019s take a quick dive into it.<\/span><\/p>\n<h3><span class=\"ez-toc-section\" id=\"What_actually_is_quinine\"><\/span><span style=\"font-family: arial, helvetica, sans-serif; The compound is a natural extract from the bark of the cinchona tree in Peru, and has been in circulation since the Jesuit missionaries brought it back to Europe in 1636. However, by the 1800s, the bitter-tasting quinine became lauded as a wonder drug in tonic waters to help with irregular heart palpitations and muscle cramps. It also has an \u201cisomer,\u201d which is a different structure or orientation of the compound that may result in vastly different properties. This isomer is <\/span><b>quinidine<\/b><span style=\"font-weight: 400;\"> and is known to also be anti-malarial and treat irregular heart conditions better than quinine. <\/span><\/span><\/p>\n<h3><span class=\"ez-toc-section\" id=\"How_does_all_this_tie_in_with_grapefruit_juice\"><\/span><strong><span style=\"font-size: 18pt; Unfortunately, the Viscount had no knowledge of this and upon drinking the grapefruit juice, he eventually died from untreated cardiac arrest.\u00a0 <\/span><\/span><\/p>\n<p><span style=\"font-family: arial, helvetica, sans-serif; font-size: 12pt;\"><span style=\"font-weight: 400;\">However, research has proven that grapefruit juice does nothing to <\/span><b>quinine<\/b><span style=\"font-weight: 400;\">. A study in New Zealand showed that healthy patients who ingested 600 mg of quinine sulfate and 200 mL of grapefruit juice did not suffer from any adverse side-effects (<\/span><a rel=\"nofollow noopener noreferrer\" target=\"_blank\" href=\"https:\/\/link.springer.com\/article\/10.1007%2Fs002280050646\"><span style=\"font-weight: 400;\">P. C. Ho et. al<\/span><\/a><span style=\"font-weight: 400;\">). The <em>American Journal of Medicine<\/em> reported a case of a 31-year-old woman with QT syndrome dying of <\/span><i><span style=\"font-weight: 400;\">torsade de pointes<\/span><\/i><span style=\"font-weight: 400;\"> due to an excessive intake of grapefruit juice and tonic water with quinidine (<\/span><a rel=\"nofollow noopener noreferrer\" target=\"_blank\" href=\"https:\/\/www.amjmed.com\/article\/S0002-9343(03)00071-8\/fulltext\"><span style=\"font-weight: 400;\">Hermans et. al 2003<\/span><\/a><span style=\"font-weight: 400;\">). But that\u2019s not all. Chemical isolation processes to separate compounds were a relatively new concept in Victorian England, so refining quinine for public use may have been quite shoddy. At the same time, it must have also been a misnomer that quinidine <\/span><i><span style=\"font-weight: 400;\">is<\/span><\/i><span style=\"font-weight: 400;\"> quinine and they were falsely advertised under the same name for the sake of profit. Consumers don\u2019t care what the name of a drug is or its respective side-effects \u2014 as long as the drug has a similar name, it\u2019s considered <\/span><i><span style=\"font-weight: 400;\">the same<\/span><\/i><span style=\"font-weight: 400;\">.
