When will you get it?<\/strong>&#8221;<\/p>\n<div>\n                                                                        <span style=\"font-weight: 400;\">Canada hasn\u2019t flattened the curve since August. New daily cases for COVID-19 are now more than twice as high as they were during the peak of the first wave. The death count is steadily climbing towards 100 new fatalities each day.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Yet as some cities and provinces go into lockdown again, an end of the pandemic appears within sight as vaccines from AstraZeneca, Moderna and Pfizer have all started their applications for approval in Canada, with more likely to follow in the coming months.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">To find out how long it might take for Health Canada to approve the vaccine candidates and when people in this country will start getting inoculated, <\/span><i><span style=\"font-weight: 400;\">Maclean\u2019s <\/span><\/i><span style=\"font-weight: 400;\">spoke with the federal health department\u2019s chief medical advisor, Dr. Supriya Sharma.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">This conversation is edited for length and clarity.<\/span><\/p>\n<hr\/>\n<h3><span class=\"ez-toc-section\" id=\"Q_There_are_currently_three_COVID-19_vaccine_candidates_that_have_started_submission_to_Health_Canada_for_approval_Where_do_they_all_stand_right_now_in_the_process\"><\/span><b>Q: There are currently three COVID-19 vaccine candidates that have started submission to Health Canada for approval. Where do they all stand right now in the process?\u00a0<\/b><span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p><span style=\"font-weight: 400;\"><strong>A:<\/strong> We have three vaccines under review. All of them started at different times in October. They came in under the interim order process\u2014one of the regulatory mechanisms we use in public health emergencies as a rapid way for a regulatory pathway. One of the flexibilities it allow<\/span><span style=\"font-weight: 400;\">s for is a rolling submission.\u00a0<\/span><\/p>\n<h3><span class=\"ez-toc-section\" id=\"Q_And_what_is_a_rolling_submission\"><\/span><strong>Q:\u00a0And what is a rolling submission?<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p><span style=\"font-weight: 400;\"><strong>A:<\/strong> Normally, a company takes all the information it has\u2014pre-clinical [tests], animal tests, all information from the clinical trials, the manufacturing information, the site of manufacturing already licensed\u2014all of that comes in as one package, and we start the review when we get it. In a rolling review, we allow companies to give us data as it becomes available.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The three under review right now are at different stages, in terms of what information we have. For some, it\u2019s just the pre-clinical information, so just the animal data. For some we have some clinical data. And for others we are about to have a full clinical package.<\/span><\/p>\n<h3><span class=\"ez-toc-section\" id=\"Q_Do_you_need_everything_for_the_stamp_of_approval\"><\/span><b>Q: Do you need everything for the stamp of approval?<\/b><span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p><span style=\"font-weight: 400;\"><strong>A:<\/strong> In terms of the final assessment\u2014if it can be authorized or not\u2014we need all the data.<\/span><\/p>\n<h3><span class=\"ez-toc-section\" id=\"Q_Have_the_applicants_notified_Health_Canada_when_the_full_detailed_data_will_be_available\"><\/span><b>Q: Have the applicants notified Health Canada when the full, detailed data will be available?<\/b><span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p><span style=\"font-weight: 400;\"><strong>A:<\/strong> Part of the requirements of the rolling submission is you have to be at a certain stage of development\u2014good, promising data from Phase 1 and Phase 2 studies, and have Phase 3 started. When they give us the\u00a0[initial tranche of] data, they provide an approximate timeline for when they\u2019ll give us the other parts of the submission.\u00a0<\/span><\/p>\n<h3><span class=\"ez-toc-section\" id=\"Q_And_when_will_these_companies_have_all_their_data_submitted_This_year_January_February\"><\/span><b>Q: And when will these companies have all their data submitted? This year? January? February?<\/b><span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p><span style=\"font-weight: 400;\"><strong>A:<\/strong> That\u2019s difficult to predict, and everything we\u2019re provided as part of this is confidential.\u00a0<\/span><span style=\"font-weight: 400;\">For the 10 vaccines in Phase 3 trials\u2014there is a wave of information on clinical trials that we were expecting in November and December. We\u2019re seeing those now with AstraZeneca, Pfizer and Moderna. We have another wave of data coming in December into January. And then another wave in the spring.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Everything seems to be on track, so far. We\u2019re expecting complete data packages for some vaccines at the end of November and into December.\u00a0<\/span><\/p>\n<h3><span class=\"ez-toc-section\" id=\"Q_Are_doses_being_made_for_the_Canadian_market_in_anticipation_of_approval\"><\/span><strong>Q:<\/strong><b> Are doses being made for the Canadian market in anticipation of approval?<\/b><span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p><span style=\"font-weight: 400;\"><strong>A:<\/strong> It depends on where these companies are for manufacturing and development.\u00a0\u00a0<\/span><span style=\"font-weight: 400;\">Some manufacturers are still getting their sites licensed. Others are starting to do some manufacturing. A few are starting the scale-up process.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">There will be vaccines destined for the Canadian market that may or may not be the same vaccine destined for the U.S. market, or the same as the European market. The companies make those decisions. They\u2019ve got different manufacturing sites that need to have the appropriate approvals. They need to be inspected and be in compliance with good manufacturing standards.<\/span><\/p>\n<h3><span class=\"ez-toc-section\" id=\"Q_So_the_Pfizer_vaccine_going_to_Americans_lets_say_might_not_be_the_same_as_the_Pfizer_vaccine_going_to_Canadians\"><\/span><strong>Q:<\/strong><b> So the Pfizer vaccine going to Americans, let\u2019s say, might not be the same as the Pfizer vaccine going to Canadians?\u00a0<\/b><span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p><span style=\"font-weight: 400;\"><strong>A:<\/strong> The clinical data is the same. The studies are the same. It\u2019s basically the same thing, but because of the complexity of the vaccine manufacturing process, any small shift in the manufacturing can result in changes in the vaccine.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Then the question is \u201chow different?\u201d It\u2019s just making sure that, in that facility, the first dose that comes off the line is the exact same as the one millionth dose. If it\u2019s made in a different facility, from a regulatory perspective, it\u2019s a different vaccine because the conditions in which it is made are different. It\u2019s still the Pfizer vaccine or the Moderna vaccine.<\/span><\/p>\n<h3><span class=\"ez-toc-section\" id=\"Q_But_isnt_this_so_tightly_regulated_that_the_vaccines_would_have_to_be_the_exact_same\"><\/span><b>Q: But isn\u2019t this so tightly regulated that the vaccines would have to be the exact same?<\/b><span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p><span style=\"font-weight: 400;\"><strong>A:<\/strong> To use an analogy, say you have the same restaurant in Europe as the United States. You have different chefs. But they have the same menu, have the same recipes and make the same food. But you have to make sure each of them passes inspection. In the process of making that meal\u2014even if they follow the same directions\u2014there will be little differences between the two.\u00a0<\/span><\/p>\n<h3><span class=\"ez-toc-section\" id=\"Q_This_sounds_like_were_comparing_a_Big_Mac_and_Le_Big_Mac_in_France\"><\/span><strong>Q:<\/strong><b> This sounds like we\u2019re comparing a Big Mac and Le Big Mac in France.<\/b><span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p><span style=\"font-weight: 400;\"><strong>A:<\/strong> Yeah.<\/span><\/p>\n<h3><span class=\"ez-toc-section\" id=\"Q_Will_we%E2%80%94or_can_we%E2%80%94make_any_of_the_three_current_vaccines_seeking_approval_in_Canada_ourselves_under_a_licence\"><\/span><b>Q: Will we\u2014or can we\u2014make any of the three current vaccines seeking approval in Canada ourselves, under a licence?\u00a0<\/b><span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p><span style=\"font-weight: 400;\"><strong>A:<\/strong> For these first three [vaccines applicants], there are no plans to manufacture them in Canada. That\u2019s not unusual. The majority of vaccines we get in Canada are imported. The one vaccine that has been subject to the advance-purchase agreement that will be manufactured in Canada is the Medicago vaccine. <\/span><span style=\"font-weight: 400;\">But the ones we are looking at for reviews would not be manufactured in Canada\u2014at least not at the beginning.\u00a0<\/span><\/p>\n<h3><span class=\"ez-toc-section\" id=\"Q_Canadas_not_the_only_country_with_plenty_of_pre-orders_for_some_of_these_vaccines_Where_are_we_on_the_list_of_which_country_gets_them_next\"><\/span><b>Q: Canada\u2019s not the only country with plenty of pre-orders for some of these vaccines. Where are we on the list of which country gets them next?<\/b><span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p><span style=\"font-weight: 400;\"><strong>A:<\/strong> Each contract is different, and for that I\u2019d have to refer you to Public Services and Procurement Canada. The idea was\u2014in discussion with the companies\u2014we\u2019d get an idea of which vaccines were of interest, what their manufacturing capacities would be. Then you can decide on the amount for the advanced purchase agreement. <\/span><\/p>\n<p><span style=\"font-weight: 400;\">We have more vaccine orders than we would need overall, because at the time we didn\u2019t know which vaccines would progress through development. There\u2019s still a risk some might not make it through the review process. But that is the financial risk; we\u2019re not taking additional risk on the safety or efficacy of the vaccine.<\/span><\/p>\n<h3><span class=\"ez-toc-section\" id=\"Q_Once_approved_vaccines_are_shipped_to_Canada_is_their_distribution_handled_by_each_province_and_territory_Is_it_under_their_authority_to_decide_who_gets_it_first_Like_say_long-term_care_homes\"><\/span><b>Q: Once approved vaccines are shipped to Canada, is their distribution handled by each province and territory? Is it under their authority to decide who gets it first? Like, say, long-term care homes?<\/b><span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Ultimately, it is the decision of the provinces and territories, but there are other things done at the federal level to help inform that process. There\u2019s the ordering of the vaccines\u2014we bring in provinces and territories to assess their supply chains and capacity for distribution. Then we have the national advisory committee on immunization, an independent, external group who gives us advice on who the priority groups should be.\u00a0<\/span><\/p>\n<h3><span class=\"ez-toc-section\" id=\"Q_Once_Health_Canada_gives_its_approval_will_we_have_vaccines_ready_for_Canadians_the_next_day_Or_is_there_a_sense_of_how_long_it_will_take_between_regulatory_approval_and_when_people_start_getting_vaccinated\"><\/span><b>Q: Once Health Canada gives its approval, will we have vaccines ready for Canadians the next day? Or is there a sense of how long it will take between regulatory approval and when people start getting vaccinated?<\/b><span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p><span style=\"font-weight: 400;\"><strong>A:<\/strong> There likely won\u2019t be a large gap between when we get authorization and when vaccines start to get shipped and the first ones come through.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">As we go further in with some of the other vaccines, one of the other provisions is the ability to import vaccines to Canada before it gets authorization. In those cases, where there is more lead time, we would allow vaccines to be brought to Canada, stored on Canadian soil and only distributed once it has Health Canada authorization.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">For the first vaccines, we likely won\u2019t have time for that. Plus, the first couple of vaccines have a more complicated distribution, with the cold chain and the extra-cold chains for those\u00a0<\/span><span style=\"font-weight: 400;\">that have to be stored at very low temperatures. In those cases, they tend to ship it directly to where it\u2019d be distributed.<\/span><\/p>\n<h3><span class=\"ez-toc-section\" id=\"Q_When_you_say_it_wont_be_a_large_gap_does_that_mean_days_Weeks\"><\/span><b>Q: When you say it won\u2019t be a large gap, does that mean days? Weeks?<\/b><span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p><span style=\"font-weight: 400;\"><strong>A:\u00a0<\/strong><\/span>We\u2019re hoping that by January, at the earliest, we may be starting to immunize people. That means authorization is done. Logistics is sorted. And, most importantly, there has to be a vaccine to be distributed.<\/p>\n<p><span style=\"font-weight: 400;\">We\u2019re all gearing towards having vaccinations happen in the first quarter of 2021.\u00a0But it can happen earlier if things move faster.
