Three-quarters of adults in a 2018\u00a0<a class=\"icon none\" href=\"https:\/\/www.aarp.org\/research\/topics\/community\/info-2018\/2018-home-community-preference.html\" target=\"_blank\" rel=\"noopener\">AARP survey<\/a>\u00a0said they wanted to remain in their homes, but only 59% thought they would be able to do so. If remaining at home is your preference, here are nine steps you can take:<\/p>\n<div class=\"paywall\">\n<h2><span class=\"ez-toc-section\" id=\"1_Prevent_falls\"><\/span><strong>1. Prevent falls<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Falls are the most common cause\u00a0of<a class=\"icon none\" href=\"https:\/\/www.ncoa.org\/center-for-healthy-aging\/falls-resource-center\/falls-prevention-tools-and-resources\/falls-prevention-older-adults-caregivers\/\" target=\"_blank\" rel=\"noopener\">\u00a0nonfatal, trauma-related hospital admissions<\/a>\u00a0for older adults.\u00a0According to the Centers for Disease Control and Prevention, one-quarter of Americans 65 and older fall every year.<\/p>\n<p>A fall can\u00a0trigger a decline in functioning \u201cwhich may affect your ability to remain independent,\u201d explains Liz Barlowe, aging life care manager with Barlowe &amp; Associates in Seminole, Fla.<\/p>\n<p>Falls result from muscle weakness, vision problems, medication side effects, poor nutrition, chronic health conditions and unsafe home environments. The National Council on Aging has a\u00a0<a class=\"icon none\" href=\"https:\/\/www.ncoa.org\/healthy-aging\/falls-prevention\/falls-free-checkup\/\" target=\"_blank\" rel=\"noopener\">quiz<\/a>\u00a0to determine whether you are at risk.\u00a0To\u00a0<a class=\"icon none\" href=\"https:\/\/www.ncoa.org\/healthy-aging\/falls-prevention\/preventing-falls-tips-for-older-adults-and-caregivers\/6-steps-to-protect-your-older-loved-one-from-a-fall\/\" target=\"_blank\" rel=\"noopener\">prevent falls<\/a>:<\/p>\n<ul class=\"articleList\">\n<li><strong>Talk to your doctor about weakness, dizziness or vision problems, management of chronic health conditions and medication side effects<\/strong>. The physician may have some helpful ideas.<\/li>\n<li><strong>Have your home evaluated by an occupational therapist for issues with lighting, <a href=\"https:\/\/buradabiliyorum.com\/en\/category\/trip-and-travel\/\" data-internallinksmanager029f6b8e52c=\"10\" title=\"Trip &amp; Travel\" target=\"_blank\" rel=\"noopener\">trip<\/a>ping hazards, stairs and bathroom safety<\/strong>. This pro might offer practical safety suggestions.<\/li>\n<li><strong>Keep moving.<\/strong>\u00a0\u201cDo regular strength and balance exercises like Tai-chi,\u201d suggests Dr. Nirmala Gopalan, medical director of the geriatrics clinic at Santa Clara Valley Medical Center in San Jose, Calif. Continued mobility is essential in preventing falls. Once you fall, it\u2019s common to be fearful, but reducing your activity just makes you more prone to falls. Instead, learn how to modify your movements and environment to make activity safe.<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"2_Work_with_experts\"><\/span><strong>2. Work with experts<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Switching to a geriatrician for primary care, for instance, helps preserve independence.<\/p>\n<p>Geriatricians are trained to manage the complexity of multiple complicated health concerns \u201cand know the difference between normal aging and more serious illness,\u201d says Gopalan.<\/p>\n<p><strong>Also see:\u00a0<\/strong>We want to buy a home on 10 wide-open acres and live on $50,000 a year \u2014 where should we retire?<\/p>\n<p>\u201cAn older person\u2019s body metabolizes medications differently. Geriatricians are trained to recognize side effects and drug interactions in seniors and manage polypharmacy [the simultaneous use of multiple drugs by a single patient] safely,\u201d Gopalan says.<\/p>\n<p>Move to a geriatrician \u201cwhen your health is starting to impair your mental or physical well-being and the activities of daily life,\u201d advises Dr. Alicia Arbaje, associate professor of medicine at Johns Hopkins University School of Medicine in Baltimore.<\/p>\n<p>\u201cSome reasons might be: (you\u2019re) in and out of the hospital a lot, on many medications, or (having) difficulty with mobility, memory or medication,\u201d adds Arbaje, who is also chair of the American Geriatrics Society\u2019s public education committee.<\/p>\n<p><a class=\"icon none\" href=\"https:\/\/www.aginglifecare.org\/ALCAWEB\/What_is_Aging_Life_Care\/ALCAWEB\/What_is_Aging_Life_Care\/What_you_need_to_know.aspx\" target=\"_blank\" rel=\"noopener\">Geriatric care managers<\/a>\u00a0are also key. They identify what you need help with and assist in finding <a href=\"https:\/\/buradabiliyorum.com\/en\/category\/download-scripts-themes-apps\/\" data-internallinksmanager029f6b8e52c=\"9\" title=\"Download Scripts &amp; Themes &amp; Apps\" target=\"_blank\" rel=\"noopener\">app<\/a>ropriate services.<\/p>\n<p>Kizzy Chambers, a professional care manager and licensed clinical <a href=\"https:\/\/buradabiliyorum.com\/en\/category\/social-mediaa\/\" data-internallinksmanager029f6b8e52c=\"1\" title=\"Social Media\" target=\"_blank\" rel=\"noopener\">social<\/a> worker with RR Care Management, in Orlando, Fla., recommends \u201cconsulting with a care manager as soon as there is a concern or when a family member is feeling overwhelmed with caring for their loved ones.\u201d<\/p>\n<p>For referrals, \u201ccall your local Area Agency on Aging,\u201d says Emily Greenfield, associate professor of social work at Rutgers University in New Brunswick, N.J. You can also use the federal government\u2019s\u00a0<a class=\"icon none\" href=\"https:\/\/eldercare.acl.gov\/Public\/Index.aspx\" target=\"_blank\" rel=\"noopener\">Eldercare Locator<\/a>.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"3_Stay_mentally_active\"><\/span><strong>3. Stay mentally active<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Maintaining brain health is key and helps you remain at home.<\/p>\n<p>A<a class=\"icon none\" href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4417099\/\" target=\"_blank\" rel=\"noopener\">\u00a0study<\/a>\u00a0in the journal Brain Imaging and Behavior\u00a0showed that mentally stimulating <a href=\"https:\/\/buradabiliyorum.com\/en\/category\/game\/\" data-internallinksmanager029f6b8e52c=\"7\" title=\"Game\" target=\"_blank\" rel=\"noopener\">game<\/a>s and activities preserved cognitive functioning in older adults. And a\u00a0Harvard Medical School<a class=\"icon none\" href=\"https:\/\/www.health.harvard.edu\/mind-and-mood\/the-thinking-on-brain-games\" target=\"_blank\" rel=\"noopener\">\u00a0study<\/a>\u00a0showed that thinking skills, which decrease with age, can be sharpened with brain games.<\/p>\n<p>\u201cThink of your brain like a muscle. You need to alter your exercise programs to ensure you are utilizing different muscles to develop your body,\u201d says Barlowe.<\/p>\n<p>To keep your brain in peak condition, Arbaje recommends a healthy diet including fruits and vegetables.<\/p>\n<p>Gopalan says daily physical activity, fresh air and sunlight \u201care very stimulating to the brain\u201d and that the key is always to \u201cfocus on enjoyment and not achievement.\u201d<\/p>\n<h2><span class=\"ez-toc-section\" id=\"4_Access_support_services\"><\/span><strong>4. Access\u00a0support\u00a0services<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>To remain at home, \u201cit is important for people to feel independent and self-sufficient,\u201d says Arbaje. To do that, \u201cbe open to the idea of outsourcing services,\u201d recommends Barlowe.<\/p>\n<p>Getting groceries delivered can help. If you feel intimidated by apps and online platforms, Instacart offers dedicated \u201c<a class=\"icon none\" href=\"https:\/\/www.instacart.com\/help\/section\/360007797972\/360052851231\" target=\"_blank\" rel=\"noopener\">senior support specialists<\/a>\u201d who help customers order by phone.\u00a0Caregivers can arrange delivery of restaurant meals via DoorDash or Uber\u00a0UBER,\u00a0-0.69%\u00a0Eats, or frozen premade meals through services such as Freshly. Meals on Wheels is also an option.<\/p>\n<p>Amazon\u00a0AMZN,\u00a00.79%\u00a0has an automatic shipping option where family members can place needed items for their loved ones on a monthly auto-ship schedule. \u201cMany of the managed care Medicare insurance plans [Medicare Advantage] allow insured individuals to order medical and nonmedical supplies, such as gloves, over-the-counter medications and adult briefs,\u201d says Chambers. Most drugstores also deliver.<\/p>\n<p><strong>You might like:\u00a0<\/strong>Growing old and the \u2018secret Japanese art\u2019 of gratitude<\/p>\n<p>For patients who need nursing home-level care,\u00a0PACE programs\u00a0(Programs of All-Inclusive Care for the Elderly) provide in-home service such as occupational therapy, physical therapy, meals, lab services, transportation to medical appointments and more.<\/p>\n<p>State\u00a0Area Agencies on Aging\u00a0can connect older residents with in-home services such as chore assistance and meal delivery.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"5_Discuss_medical_care_reductions\"><\/span><strong>5. Discuss medical care reductions<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>It sounds counterintuitive to reduce medical care as you age, but \u201cwe often rely too much on too many healthcare providers, tests and procedures,\u201d says Arbaje. \u201cThese have complications, especially medications.\u201d<\/p>\n<p>Cutting back is \u201cnot a refusal of care \u2014 rather an evaluation of the burdens versus the benefits of specific care,\u201d says Dr. Tracy English, a nurse practitioner at Long Life Care Management in McDonough, Ga.<\/p>\n<p>For example, the American Medical Association recommends against routine colonoscopies\u00a0for patients over age 75\u00a0because of the risks involved. Some studies show statins (prescribed for high cholesterol) can have\u00a0<a class=\"icon none\" href=\"https:\/\/www.mayoclinic.org\/diseases-conditions\/high-blood-cholesterol\/in-depth\/statins\/art-20045772\" target=\"_blank\" rel=\"noopener\">more side effects and fewer benefits<\/a>\u00a0for older adults. Benzodiazepines (used for anxiety, depression and seizures)\u00a0<a class=\"icon none\" href=\"https:\/\/www.webmd.com\/mental-health\/addiction\/benzodiazepine-abuse#1\" target=\"_blank\" rel=\"noopener\">can cause\u00a0<\/a>memory issues in older patients.<\/p>\n<p>Barlow advises taking time to consider what your physician is ordering or recommending. \u201cDiscuss with your doctor how you feel about your care as you age and what is important to you \u2013 independence, optimal functioning and quality of life,\u201d she says.<\/p>\n<p>Arbaje suggests older adults \u201crevisit the risks and benefits (of treatments) often, as these change over time.\u201d<\/p>\n<h2><span class=\"ez-toc-section\" id=\"6_Maintain_relationships\"><\/span><strong>6. Maintain relationships<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Social and family connections\u00a0support mental health, such as the ability to manage stress and prevent depression. Tam Perry, associate professor of social work at Wayne State University in Detroit, suggests older adults find ways to both give and receive support. \u201cHelping other people can be just as important and sometimes even more, for our well-being,\u201d she says.<\/p>\n<p>Family relationships are key to managing the challenges of remaining at home.\u00a0A joint report\u00a0by the National Alliance for Caregiving and AARP found that 89% of caregivers for older adults are family members.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"7_Stay_flexible\"><\/span><strong>7. Stay flexible<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>\u201cChange is necessary and rather than it being forced on you, think of taking the lead,\u201d says Barlowe. \u201cKeep your eye on your goal of living independently and be open to possibilities that will help you.\u201d<\/p>\n<p>A<a class=\"icon none\" href=\"https:\/\/www.sciencedaily.com\/releases\/2016\/04\/160420131531.htm\" target=\"_blank\" rel=\"noopener\">\u00a0study<\/a>\u00a0in Cell Press found aging causes the brain to become less adaptive to change. Older people can\u00a0become resistant\u00a0to things like taking new medications or making home modifications. (A separate study found that noncompliance with medication led to 23% of all nursing home admissions.)<\/p>\n<p>Studies also show\u00a0home modifications\u00a0allow older adults to maintain their level of activity for at least two years. So, remaining open to these is key.<\/p>\n<p>Additionally, Barlowe points out, \u201cI have met more people that out of vanity refuse a walker but will walk through their home holding on to furniture because they are so unsteady. The use of a supportive device may allow you to safely remain independent.\u201d<\/p>\n<h2><span class=\"ez-toc-section\" id=\"8_Start_saving_money\"><\/span><strong>8. Start saving money<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Remaining at home can be expensive. \u201c[In-home] caregiving expenses are very high and a significant factor to be able to continue to stay at home when someone becomes dependent with multiple medical conditions,\u201d says Gopalan.<\/p>\n<p>Rates for nonmedical aides (for things like housework, meals and errands) range from $16 to $28 per hour depending on where you live. Home healthcare aides\u00a0may cost\u00a0up to $30 per hour.<\/p>\n<p><strong>See:\u00a0<\/strong>All the ways older people mismanage their money \u2014 and how to avoid them<\/p>\n<p>Home modifications can range from inexpensive changes (railings or flashing doorbells) to major expenses such as stair lifts.\u00a0Medicare does not cover\u00a0home modifications.\u00a0Medicaid may provide some coverage, but each state\u2019s program is different.<\/p>\n<p>Start saving for these costs now. \u201cOlder people who have paid off a mortgage and invested in adaptations to minimize effects of disability are best off,\u201d says Stephen Albert, professor at University of Pittsburgh School of Public Health and editor of the journal Innovation in Aging<em>.<\/em><\/p>\n<h2><span class=\"ez-toc-section\" id=\"9_Have_a_plan\"><\/span><strong>9. Have a plan<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Staying in your own home requires thinking ahead. If you have chronic conditions, talk with your doctor to understand how the disease could progress, so you can anticipate how to manage it in the future.<\/p>\n<p>Plan\u00a0for how you will get help when needed. \u201cPersonal emergency response (PER) systems [also called medical alert systems] are effective and efficient, says Chambers. \u201cPERs allow for quicker response time by first responders and for medical attention to be provided,\u201d she adds.<\/p>\n<p><strong>See:\u00a0<\/strong>Do you have a disaster plan for your parents?<\/p>\n<p>Arbaje says the most important thing is to talk to family \u201cabout what you would want if you were to get sick \u2014 who will take care of you, who will take care of the pets and who will take care of your spouse.\u201d<\/p>\n<p><em>Brette Sember\u00a0is the author of many books about divorce, child custody, business, health, food, and travel.
