What does that mean for those who&#8217;ve already had one?\n                        <\/div>\n<div>\n                                                                        <span style=\"font-weight: 400;\">Virtually all of the 2.3 million doses of AstraZeneca COVID-19 vaccine in Canada are in the arms of Canadians. Ontario has just 50,000 doses left out of its 900,000 allotment. Alberta <\/span><span style=\"font-weight: 400;\">has 8,200 doses remaining after the province gave out shots to 225,000 Albertans, while <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/montreal.ctvnews.ca\/after-ontario-suspends-astrazeneca-first-doses-quebec-says-it-has-no-similar-plans-1.5423598?cid=sm%3Atrueanthem%3Actvmontreal%3Atwitterpost&amp;taid=609b5d5412982200014dd32d&amp;utm_campaign=trueAnthem%3A+Trending+Content&amp;utm_medium=trueAnthem&amp;utm_source=twitter\"><span style=\"font-weight: 400;\">Quebec has all but exhausted its supply<\/span><\/a><span style=\"font-weight: 400;\">. <\/span><span style=\"font-weight: 400;\">So when Ontario and Alberta announced on Tuesday they were going to stop giving AstraZeneca vaccine as first doses, a lot of people im<a href=\"https:\/\/buradabiliyorum.com\/en\/category\/social-mediaa\/\" data-internallinksmanager029f6b8e52c=\"1\" title=\"Social Media\" target=\"_blank\" rel=\"noopener\">media<\/a>tely asked the same question: What the heck h<a href=\"https:\/\/buradabiliyorum.com\/en\/category\/download-scripts-themes-apps\/\" data-internallinksmanager029f6b8e52c=\"9\" title=\"Download Scripts &amp; Themes &amp; Apps\" target=\"_blank\" rel=\"noopener\">app<\/a>ens next?\u00a0<\/span><\/p>\n<h2><span class=\"ez-toc-section\" id=\"Why_arent_provinces_giving_first_doses_of_AstraZeneca_vaccine\"><\/span><b>Why aren\u2019t provinces giving first doses of AstraZeneca vaccine?\u00a0<\/b><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Alberta\u2019s medical officer of health <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.cbc.ca\/news\/canada\/edmonton\/astrazeneca-covid-19-vaccine-alberta-1.6021739\"><span style=\"font-weight: 400;\">Dr. Deena Hinshaw explained<\/span><\/a><span style=\"font-weight: 400;\"> that one of the reasons was that the province doesn\u2019t know when future supplies will come and wants to conserve its remaining supplies for second doses. Ontario\u2019s medical officer of health <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/news.ontario.ca\/en\/statement\/1000103\/ontario-pauses-administration-of-astrazeneca-vaccine\"><span style=\"font-weight: 400;\">Dr. David Williams<\/span><\/a><span style=\"font-weight: 400;\"> said his province was <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.theglobeandmail.com\/politics\/article-ontario-stops-first-doses-of-astrazeneca-covid-19-vaccine\/\"><span style=\"font-weight: 400;\">postponing first doses \u201cout of the abundance of caution\u201d<\/span><\/a><span style=\"font-weight: 400;\"> because of rising numbers of a <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/covid19-sciencetable.ca\/sciencebrief\/risk-of-vaccine-induced-thrombotic-thrombocytopenia-vitt-following-the-astrazeneca-covishield-adenovirus-vector-covid-19-vaccines\/\"><span style=\"font-weight: 400;\">potentially fatal blood clot condition known as <\/span><span style=\"font-weight: 400;\">vaccine-induced thrombotic thrombocytopenia (VITT) <\/span><\/a><span style=\"font-weight: 400;\">reported in those given the AstraZeneca vaccine.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Though experts explain that the risk of getting VITT is extremely rare, a higher number of cases have been<\/span> <span style=\"font-weight: 400;\">detected through post-vaccination surveillance systems around the world. So, while Canada\u2019s rate had been around 1 in 100,000 as of the end of April, some European nations are now reporting a risk of VITT at around 1 in 26,000.\u00a0<\/span><\/p>\n<h2><span class=\"ez-toc-section\" id=\"Why_is_the_VITT_risk_ratio_changing\"><\/span><b>Why is the VITT risk ratio changing?\u00a0<\/b><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p><span style=\"font-weight: 400;\">First, it can take a while for possible adverse events to work their way through national and international surveillance reporting systems and for researchers to find patterns in the data. Around 20 million doses of AstraZeneca were administered before <\/span><span style=\"font-weight: 400;\">the first cases of VITT were publicly identified in mid-March<\/span><span style=\"font-weight: 400;\">. And it took around <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.cbc.ca\/news\/health\/canada-first-case-blood-clots-astrazeneca-oxford-vaccine-vipit-1.5985755\"><span style=\"font-weight: 400;\">six weeks for Canada\u2019s first case to be reported<\/span><\/a><span style=\"font-weight: 400;\"> in mid-April. Now, as front line medical staff are looking for symptoms of VITT, cases are being reported faster, and the ratios are updated. For instance, reports of new cases of VITT in Ontario<\/span> <span style=\"font-weight: 400;\">were received by the government late on Monday, officials explained. And that new data changed the risk ratio from one case of VITT per 100,000 doses to one in 58,000.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In Scandinavia, which has a world-famous system for gathering and interpreting health data, the ratios are even lower. Dr. David Fisman, an epidemiologist at the University of Toronto, told <\/span><i><span style=\"font-weight: 400;\">Maclean\u2019s <\/span><\/i><span style=\"font-weight: 400;\">that Norway, which had reported a ratio of one case of VITT to every 26,000 administered doses has now seen that ratio drop to one in 16,000.\u00a0<\/span><\/p>\n<h2><span class=\"ez-toc-section\" id=\"What_is_the_risk_of_VITT_in_a_second_dose_of_AstraZeneca\"><\/span><b>What is the risk of VITT in a second dose of AstraZeneca?\u00a0<\/b><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Right now, the risk of VITT associated with second doses of the AstraZeneca vaccine is based on data from the United Kingdom, which has administered <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.gov.uk\/government\/publications\/coronavirus-covid-19-vaccine-adverse-reactions\/coronavirus-vaccine-summary-of-yellow-card-reporting#yellow-card-reports\"><span style=\"font-weight: 400;\">tens of millions of doses<\/span><\/a><span style=\"font-weight: 400;\">. According to data from Britain, that risk is <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.gov.uk\/government\/publications\/coronavirus-covid-19-vaccine-adverse-reactions\/coronavirus-vaccine-summary-of-yellow-card-reporting#yellow-card-reports\"><span style=\"font-weight: 400;\">roughly one in a million<\/span><\/a><span style=\"font-weight: 400;\"> for those who have received second doses, (<\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.gov.uk\/government\/publications\/coronavirus-covid-19-vaccine-adverse-reactions\/coronavirus-vaccine-summary-of-yellow-card-reporting#yellow-card-reports\"><span style=\"font-weight: 400;\">six cases in 5.9 million second doses<\/span><\/a><span style=\"font-weight: 400;\">.)<\/span><\/p>\n<h2><span class=\"ez-toc-section\" id=\"Will_people_be_assigned_AstraZeneca_or_another_vaccine_for_their_second_doses_or_will_they_get_to_choose\"><\/span><b>Will people be assigned AstraZeneca or another vaccine for their second doses, or will they get to choose?\u00a0<\/b><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p><span style=\"font-weight: 400;\">That issue is being worked on right now by governments. As an Ontario official explained in a briefing to media, there are<\/span> <span style=\"font-weight: 400;\">a lot of factors to weigh, including the shipment schedule for second doses, the 16-week dosing interval, concerns about VITT for those getting second doses as well as an upcoming study from Britain that is examining the safety and effectiveness of mixing-and-matching COVID-19 vaccines.\u00a0<\/span><\/p>\n<h2><span class=\"ez-toc-section\" id=\"Explain_this_mix-and-match_option\"><\/span><b>Explain this mix-and-match option?\u00a0\u00a0<\/b><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Earlier this year, Oxford University started a study to see what would happen if people were given one dose of a viral vector vaccine (AstraZeneca) and another dose of a mRNA one (Pfizer). <\/span><span style=\"font-weight: 400;\">It\u2019s called the<\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/comcovstudy.org.uk\/home\"><span style=\"font-weight: 400;\"> Comparing COVID-19 Vaccine Schedule Combinations trial<\/span><\/a><span style=\"font-weight: 400;\">, or the Com-COV vaccine trial in short. And as more vaccines were approved, Oxford started a second study (<\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/comcovstudy.org.uk\/about-com-cov2\"><span style=\"font-weight: 400;\">Com-COV2<\/span><\/a><span style=\"font-weight: 400;\">) examining combinations of even more viral and mRNA options, including Moderna.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Theoretically, using a mix-and-match system may produce a better immune response. \u201cThink of vaccination as presenting our immune system with a red flag\u2014here is something you should be on the lookout for,\u201d <\/span><span style=\"font-weight: 400;\">Dr. Alan Bernstein, CEO of CIFAR, a Toronto-based global research organization, told <\/span><i><span style=\"font-weight: 400;\">Maclean\u2019s<\/span><\/i><span style=\"font-weight: 400;\"> in February<\/span><span style=\"font-weight: 400;\">. \u201cAnd so, if you go with the mRNA vaccine first, you\u2019re going to activate one branch of our immune system.\u201d Bernstein suggests thinking of this first vaccine as our \u201cair force.\u201d Then, he says, you introduce one of the viral vector vaccines a couple of weeks later, and it tells the immune system: \u201c\u2018You saw this guy before, but I\u2019m going to present him in a different format,\u2019\u201d says Bernstein. \u201cAnd so it\u2019s going to activate a different branch of our immune system. Think of it as our navy. And so now you have the air force and the navy activated and so you\u2019ve got a double threat against this incoming virus.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Results of the initial Com-COV study looking at AstraZeneca and Pfizer are due any day. Until they can see those results, governments have to assume that those who got first doses of AstraZeneca will get second doses of the same vaccine.\u00a0<\/span><\/p>\n<h2><span class=\"ez-toc-section\" id=\"When_are_more_AstraZeneca_doses_arriving\"><\/span><b>When are more AstraZeneca doses arriving?\u00a0<\/b><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p><span style=\"font-weight: 400;\">On Wednesday, just after Maj.-Gen. Dany Fortin announced that 655,000 doses of AstraZeneca will arrive at some point in May, Ontario revealed that the shipment would be distributed to the provinces next week, with Ontario\u2019s share being 254,500 doses. As well, all those doses arriving in Ontario will be allocated for second doses.\u00a0<\/span><\/p>\n<h2><span class=\"ez-toc-section\" id=\"Can_we_speed_up_the_administration_of_second_doses\"><\/span><b>Can we speed up the administration of second doses?<\/b><span style=\"font-weight: 400;\">\u00a0<\/span><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p><span style=\"font-weight: 400;\">There is a clock ticking on those AstraZeneca doses still in vials. They arrived in a shipment of 1.5 million doses from the United States, and, in an April 5 press conference, Fortin explained that two-thirds of the doses in that shipment expire in May with the rest expiring in June. (On Wednesday, Ontario officials confirmed the expiration timing.)\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Many Canadians are more than willing to get those second AstraZeneca doses before they have to be thrown out, if the reaction on social media is any indication. But at the same time Ontario officials announced the new shipment of AstraZeneca doses, they also reiterated that the current interval of up to 16 weeks between doses would be maintained.
