Right now, more than <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/covid19tracker.ca\/vaccinationtracker.html\"><span style=\"font-weight: 400;\">53 per cent <\/span><\/a><span style=\"font-weight: 400;\">of the country\u2019s population has received at least one dose, but only 4.6 per cent are fully vaccinated. That latter number is climbing:\u00a0<\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/art-bd.shinyapps.io\/covid19canada\/\"><span style=\"font-weight: 400;\">around one sixth<\/span><\/a><span style=\"font-weight: 400;\"> of all doses administered on May 26 were second doses. At the same time<\/span><b>, <\/b><span style=\"font-weight: 400;\">provinces are compressing the four-month interval between doses that is ticking away for Canadians, especially those who received their first shots earlier this year, while new research is emerging about the optimal time to wait.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Getting a sizable chunk of the population fully immunized is crucial for summer reopening plans. According to Canadian federal modelling, it\u2019s <\/span><span style=\"font-weight: 400;\">only when <\/span><span style=\"font-weight: 400;\">75 per cent of eligible Canadians have first doses plus 20 per cent have second doses that the Public Health Agency of Canada believes reopenings can occur without putting the health care system at risk from another wave.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">So what is the plan for getting second shots in arms?\u00a0<\/span><\/p>\n<h2><span class=\"ez-toc-section\" id=\"Possible_danger_ahead_Variants_of_concern\"><\/span><b>Possible danger ahead: Variants of concern<\/b><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p><span style=\"font-weight: 400;\">The focus on second doses is also intensifying because experts are worried about the B.1.617.2 variant of concern (VOC) that was first detected in India. A <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/khub.net\/documents\/135939561\/430986542\/Effectiveness+of+COVID-19+vaccines+against+the+B.1.617.2+variant.pdf\/204c11a4-e02e-11f2-db19-b3664107ac42\"><span style=\"font-weight: 400;\">new preprint study<\/span><\/a><span style=\"font-weight: 400;\"> by Public Health England (PHE) found that while both the Pfizer and AstraZeneca vaccines held up against B.1.617.2 compared to the established B.1.1.7 VOC, effectiveness was noticeably better after two doses compared to after one.\u00a0<\/span><\/p>\n<blockquote><p><strong>RELATED:\u00a0What we know about the COVID variant first found in India<\/strong><\/p>\n<\/blockquote>\n<h2><span class=\"ez-toc-section\" id=\"Who_is_first_in_line_for_second_doses\"><\/span><b>Who is first in line for second doses?<\/b><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p><span style=\"font-weight: 400;\">The authors of the PHE preprint study concluded that their findings \u201cwould support maximising vaccine uptake with two doses among vulnerable groups.\u201d That <a href=\"https:\/\/buradabiliyorum.com\/en\/category\/download-scripts-themes-apps\/\" data-internallinksmanager029f6b8e52c=\"9\" title=\"Download Scripts &amp; Themes &amp; Apps\" target=\"_blank\" rel=\"noopener\">app<\/a>roach has already started in Canada. Even before unveiling plans for widespread second-dose rollouts, provinces were prioritizing second doses for Indigenous populations, front-line medical workers as well as those with high-risk medical conditions such as recipients of organ transplants or people with immune disorders.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">New research has led to more at-risk people being added to the priority list. Earlier in May, <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.cmaj.ca\/content\/early\/2021\/05\/11\/cmaj.210673\"><span style=\"font-weight: 400;\">a study<\/span><\/a><span style=\"font-weight: 400;\"> in the <\/span><i><span style=\"font-weight: 400;\">Canadian Medical Association Journal<\/span><\/i><span style=\"font-weight: 400;\"> revealed that first doses of the Pfizer vaccine \u201c<\/span><span style=\"font-weight: 400;\">failed to elicit a humoral immune response in most patients receiving hemodialysis.\u201d Its authors suggested that dialysis patients be prioritized for second doses at the original 21-day interval, a recommendation<\/span> <span style=\"font-weight: 400;\">which Ontario, <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.gov.mb.ca\/covid19\/vaccine\/eligibility-criteria.html#second-dose\"><span style=\"font-weight: 400;\">Manitoba<\/span><\/a><span style=\"font-weight: 400;\"> and other jurisdictions adopted.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">As of now, 88.7 per cent of seniors 60 and older have received first doses, while only 9.3 per cent have received second doses, according to <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/twitter.com\/trevortombe\/status\/1397701033858633728?s=20\"><span style=\"font-weight: 400;\">estimates by<\/span><\/a><span style=\"font-weight: 400;\"> Trevor Tombe, an associate professor of economics at the University of Calgary. Most fully vaccinated seniors live in long-term care and retirement homes.<\/span> <span style=\"font-weight: 400;\">Seniors living at home, who were at the head of the line for first doses when vaccination efforts geared up in February and March, <\/span><span style=\"font-weight: 400;\">wound up having to wait for second shots when<\/span> <span style=\"font-weight: 400;\">the dosing interval for most Canadians was extended up to four months.\u00a0<\/span><\/p>\n<blockquote><p><strong>RELATED:\u00a0Is Canada on track to reach herd immunity?<\/strong><\/p>\n<\/blockquote>\n<p><span style=\"font-weight: 400;\">Seniors who are 70 and older should be next in line for second doses, <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.theglobeandmail.com\/opinion\/article-older-canadians-should-be-getting-their-second-vaccine-doses-right-now\/?utm_medium=Referrer:+Social+Network+\/+Media&amp;utm_campaign=Shared+Web+Article+Links\"><span style=\"font-weight: 400;\">says Dr. Nathan Stall<\/span><\/a><span style=\"font-weight: 400;\">, a geriatrician at Sinai Health in Toronto and co-author of an opinion piece in the <\/span><i><span style=\"font-weight: 400;\">Globe and Mail <\/span><\/i><span style=\"font-weight: 400;\">on May 26: \u201cOlder adults, after all, have borne the brunt of the pandemic. Despite accounting for just 12 per cent of the <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www150.statcan.gc.ca\/t1\/tbl1\/en\/tv.action?pid=1710000501\"><span style=\"font-weight: 400;\">Canadian population<\/span><\/a><span style=\"font-weight: 400;\">, people over 70 have suffered more than <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/health-infobase.canada.ca\/covid-19\/epidemiological-summary-covid-19-cases.html\"><span style=\"font-weight: 400;\">85 per cent of all COVID-19 deaths<\/span><\/a><span style=\"font-weight: 400;\"> in the country. One dose of an mRNA COVID-19 vaccine does provide <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/khub.net\/documents\/135939561\/430986542\/Early+effectiveness+of+COVID+vaccines.pdf\/ffd7161c-b255-8e88-c2dc-88979fc2cc1b?t=1614617945615\"><span style=\"font-weight: 400;\">substantial protection<\/span><\/a><span style=\"font-weight: 400;\"> against symptomatic infection, even among older adults. But the immune response to vaccination is significantly lower in older adults, and effectiveness increases from 60 to 70 per cent with one dose to 85 to 90 per cent with two.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The medical officer of health in York Region reportedly said that Ontario is reportedly planning on shortening the interval for second doses for seniors 80 and older as early as next week, according to the medical officer of health in York Region,\u00a0<\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/twitter.com\/KamilKaramali\/status\/1397922962230050822?s=20\"><span style=\"font-weight: 400;\">according to Kamil Karamal<\/span><\/a><span style=\"font-weight: 400;\">i of Global <a href=\"https:\/\/buradabiliyorum.com\/en\/category\/news\/\" data-internallinksmanager029f6b8e52c=\"2\" title=\"News\" target=\"_blank\" rel=\"noopener\">News<\/a>.\u00a0<\/span><\/p>\n<h2><span class=\"ez-toc-section\" id=\"Optimal_dosing_intervals\"><\/span><b>Optimal dosing intervals<\/b><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p><span style=\"font-weight: 400;\">A few weeks ago, researchers at the University of Birmingham in England <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.birmingham.ac.uk\/news\/latest\/2021\/05\/covid-pfizer-vaccination-interval-antibody-response.aspx\"><span style=\"font-weight: 400;\">released details<\/span><\/a><span style=\"font-weight: 400;\"> from a preprint study that showed delaying the second Pfizer dose up to 12 weeks is best for those 80 and older. In particular, such a delay from the usual <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.canada.ca\/en\/public-health\/services\/immunization\/national-advisory-committee-on-immunization-naci\/extended-dose-intervals-covid-19-vaccines-early-rollout-population-protection.html\"><span style=\"font-weight: 400;\">21-day mark<\/span><\/a><span style=\"font-weight: 400;\"> increased the \u201cantibody response 3.5-fold compared to those who had the second vaccine at three weeks,\u201d the researchers wrote, adding, \u201cThe team concluded that extending administration of the second Pfizer vaccine to 12 weeks potentially enhances and extends antibody immunity, which is believed to be important in virus neutralisation and prevention of infection.\u201d\u00a0<\/span><\/p>\n<blockquote><p><strong>RELATED:\u00a0Why some people feel COVID vaccine side effects and others don\u2019t<\/strong><\/p>\n<\/blockquote>\n<p><span style=\"font-weight: 400;\">As well, the dosing interval of the AstraZeneca vaccine, which was 28 days during its phase 3 trials was shifted to 12 weeks in Britain after researchers discovered that <\/span><span style=\"font-weight: 400;\">AstraZeneca is a \u201cslow burn\u201d vaccine<\/span><span style=\"font-weight: 400;\"> that takes time to build its strength. After a first dose, the immune response keeps growing to around 76 per cent efficacy, then, after a second shot at around 12 weeks, climbs further, reaching an efficacy of 82.4 per cent. In contrast, if doses were given less than six weeks apart, the efficacy was around 54 per cent, according <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.bmj.com\/content\/372\/bmj.n326\"><span style=\"font-weight: 400;\">to a pre-print for the <\/span><i><span style=\"font-weight: 400;\">Lancet<\/span><\/i><\/a><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A 12-week dosing interval is being used in Ontario for the vast majority of its second doses of AstraZeneca, which are now being administered to those who received a first shot in March.\u00a0<\/span><\/p>\n<h2><span class=\"ez-toc-section\" id=\"Rollout_of_second_doses_varies_by_province\"><\/span><b>Rollout of second doses varies by province<\/b><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Saskatchewan, which was the <\/span><span style=\"font-weight: 400;\">first province to unveil<\/span><span style=\"font-weight: 400;\"> its detailed reopening plan, was the provincial leader in second-dose planning as well. It will distribute second doses according to age and priority groups, and the interval will be compressed from the previous 16 weeks to 13 weeks or earlier, based on vaccine supplies. The province has provided a <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.saskatchewan.ca\/government\/health-care-administration-and-provider-resources\/treatment-procedures-and-guidelines\/emerging-public-health-issues\/2019-novel-coronavirus\/covid-19-vaccine\/vaccine-delivery-phases\"><span style=\"font-weight: 400;\">handy table<\/span><\/a><span style=\"font-weight: 400;\"> so everyone can figure out approximately when doses will be distributed. For example, those vaccinated around March 15 can expect second doses by May 31.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Quebec,<\/span> <span style=\"font-weight: 400;\">similarly has a simple online appointment portal, Clic Sant\u00e9. A source<\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/montrealgazette.com\/news\/local-news\/quebec-to-move-up-appointments-for-second-covid-19-vaccine-doses?utm_term=Autofeed&amp;utm_medium=Social&amp;utm_source=Twitter#Echobox=1621908034\"><span style=\"font-weight: 400;\"> told the <\/span><i><span style=\"font-weight: 400;\">Montreal Gazette<\/span><\/i><\/a><i><span style=\"font-weight: 400;\">:<\/span><\/i><span style=\"font-weight: 400;\"> \u201cThey are now working on the Clic Sant\u00e9 system so that you will be able to go on it and say, OK, \u2018I\u2019d like to have an appointment on such and such a date,\u2019 which is a month or two before the appointment booked previously, and it will automatically cancel that one.\u201d On Tuesday, Health Minister Christian Dub\u00e9\u00a0<\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.cbc.ca\/news\/canada\/montreal\/quebec-second-dose-1.6039781\"><span style=\"font-weight: 400;\">said that<\/span><\/a><span style=\"font-weight: 400;\"> residents of long term care homes have already received second doses while those in private seniors buildings should get second shots by the end of May. He hopes everyone can be fully vaccinated by the end of August.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">On May 21, Ontario announced that it was restarting its administration of the AstraZeneca vaccine, but only for second doses. Those who first got the vaccine during the pilot phase of the rollout between March 10 and 19 could book an appointment for a second dose starting the week of March 24. But the actual administration of those second shots was less than smooth.<\/span> <span style=\"font-weight: 400;\">On May 25, as Ontarians scoured pharmacy and government sites for information, Justin Bates, the head of the Ontario Pharmacists Association took to <a href=\"https:\/\/buradabiliyorum.com\/en\/category\/social-mediaa\/\" data-internallinksmanager029f6b8e52c=\"1\" title=\"Social Media\" target=\"_blank\" rel=\"noopener\">Twitter<\/a> to <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/twitter.com\/justjbates\/status\/1397298863401222151?s=20\"><span style=\"font-weight: 400;\">plead for patience<\/span><\/a><span style=\"font-weight: 400;\">, <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/twitter.com\/justjbates\/status\/1397231994111725568?s=20\"><span style=\"font-weight: 400;\">explaining that deliveries<\/span><\/a><span style=\"font-weight: 400;\"> to most pharmacies weren\u2019t occurring until the following day and bookings would start after that.\u00a0<\/span><\/p>\n<blockquote><p><strong>RELATED:\u00a0COVID-19 in Canada: How our battle to stop the pandemic is going<\/strong><\/p>\n<\/blockquote>\n<p><span style=\"font-weight: 400;\">Ontario\u2019s chief medical officer, Dr. David Williams sounded less certain as to when Ontario would roll out a detailed second dose plan, explaining that the province didn\u2019t want people <\/span><span style=\"font-weight: 400;\">getting second doses while others are still queuing for first doses. \u201cWe want it done as soon as we can and we\u2019ll let you know on that,\u201d he said.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Yet, second doses are going to have to start soon in Ontario, which <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/covid19tracker.ca\/provincevac.html?p=ON\"><span style=\"font-weight: 400;\">has fully vaccinated<\/span><\/a><span style=\"font-weight: 400;\"> fewer than 600,000 of its 15 million people. Ontario\u2019s <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/news.ontario.ca\/en\/release\/1000161\/ontario-releases-three-step-roadmap-to-safely-reopen-the-province\"><span style=\"font-weight: 400;\">reopening plan<\/span><\/a><span style=\"font-weight: 400;\">, which was unveiled last week, <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/news.ontario.ca\/en\/release\/1000161\/ontario-releases-three-step-roadmap-to-safely-reopen-the-province\"><span style=\"font-weight: 400;\">requires <\/span><\/a><span style=\"font-weight: 400;\">\u201c<\/span><span style=\"font-weight: 400;\">70 per cent of adults vaccinated with one dose and 20 per cent vaccinated with two doses\u201d for the province to proceed with Step 2, likely in early July.\u00a0<\/span><\/p>\n<h2><span class=\"ez-toc-section\" id=\"The_logistics_challenge_of_second_doses\"><\/span><b>The logistics challenge of second doses<\/b><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p><span style=\"font-weight: 400;\">The complications are easy to see: while provinces continue to give out as many first doses as fast as possible, they must also ensure people get the correct second doses of the right vaccine at the right time. And research into optimal intervals continues to evolve.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Other countries further ahead of Canada in administering vaccines have experienced a drop in first doses when attention shifts to second doses, in part because deliveries are finite, meaning jurisdictions have to juggle how many first doses to administer compared to time-sensitive second shots. For example, in Britain, which has its own \u201cfirst dose first\u201d plan, second doses <\/span><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.bbc.com\/news\/health-55274833\"><span style=\"font-weight: 400;\">now outnumber<\/span><\/a><span style=\"font-weight: 400;\"> first doses by a 2:1 rating.
