Credit: Shutterstock<br \/>\n            <\/figcaption><\/figure>\n<\/div>\n<\/div>\n<p>The Delta variant has taken hold in NSW and Victoria, which are both in lockdown, and now in South Australia too, which will <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.abc.net.au\/news\/2021-07-20\/south-australia-moves-into-lockdown\/100306696\">enter a lockdown tonight<\/a>.\n                                                <\/p>\n<p>                                                                                The World Health Organization has predicted Delta will become the <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.who.int\/publications\/m\/item\/weekly-epidemiological-update-on-COVID-19%E2%80%94-13-july-2021\">dominant variant globally<\/a>.<\/p>\n<p>A concerning feature of Delta is the number of children and adolescents becoming infected. Earlier in the pandemic, Australians were able to feel reassured that transmission in schools was <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.thelancet.com\/journals\/lanchi\/article\/PIIS2352-4642(20)30251-0\/fulltext\">negligible<\/a>. But Delta is already proving different, with some evidence of transmission among school children, teachers and their households emerging in Australia.<\/p>\n<p>Those who&#8217;ve been vaccinated against COVID-19 <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.wsj.com\/articles\/australias-delta-outbreak-data-show-vaccines-protect-against-hospitalization-11625904029\">have been less likely to be infected with the Delta strain<\/a>. If infected, they&#8217;re less likely to require hospitalization.  <\/p>\n<p>There&#8217;s now a sense of urgency surrounding Australia&#8217;s COVID vaccine rollout, which ranks <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.theguardian.com\/australia-news\/datablog\/ng-interactive\/2021\/jul\/16\/COVID-19-vaccine-rollout-australia-vaccination-rate-progress-how-many-people-vaccinated-percent-tracker-by-state-victoria-vic-nsw-queensland-qld-daily-live-data-stats-updates-total-number-world-ranking-distribution-schedule-tracking-chart-percentage-new-cases-today\">last among OECD countries<\/a>.<\/p>\n<p><a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/newsroom.unsw.edu.au\/news\/health\/should-australia-vaccinate-children-against-COVID-19\">Some experts<\/a> are calling for <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www1.racgp.org.au\/newsgp\/clinical\/is-australia-prepared-for-the-COVID-delta-variant\">vaccination of children and adolescents<\/a> to be a high priority as well. <\/p>\n<p>Australia&#8217;s drug regulator, the Therapeutic Goods Administration (TGA), may soon <a href=\"https:\/\/buradabiliyorum.com\/en\/category\/download-scripts-themes-apps\/\" data-internallinksmanager029f6b8e52c=\"9\" title=\"Download Scripts &amp; Themes &amp; Apps\" target=\"_blank\" rel=\"noopener\">app<\/a>rove the Pfizer and <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.tga.gov.au\/media-release\/tga-grants-provisional-determination-moderna-COVID-19-vaccine-elasomeran\">Moderna<\/a> COVID vaccines for adolescents aged 12-15.<\/p>\n<p>If the vaccines are approved for this age group, how might we undertake mass vaccination of adolescents in Australia? And who will be involved in decisions about consent?<br \/>\n                                            <!-- Google middle Adsense block --><\/p>\n<p><b>How can we vaccinate teens?<\/b><\/p>\n<p>Adolescent vaccinations have been effectively delivered in Australian states and territories <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www1.health.gov.au\/internet\/main\/publishing.nsf\/Content\/cda-cdi3702j.htm\">for many decades<\/a>, via <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/medicinetoday.com.au\/system\/files\/pdf\/MT2021-1-2-057-DAVIES.pdf\">immunization programs at schools<\/a>. <\/p>\n<p>The <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.health.gov.au\/health-topics\/immunization\/immunization-throughout-life\/national-immunization-program-schedule\">National Immunization Program<\/a> is funded by the federal government, which means vaccines listed in the schedule are provided for free.<\/p>\n<p>Vaccines are given in high schools by nurse immunisers. If a dose is missed at school, adolescents are eligible to &#8220;catch-up&#8221; via their GP (meaning the vaccine is still free).<\/p>\n<p>The current schedule for adolescents includes vaccines against <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.health.gov.au\/health-topics\/hpv-human-papillomavirus\">HPV (human papillomavirus)<\/a>, the &#8220;dTpa&#8221; (<a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.health.gov.au\/health-topics\/diphtheria\">diphtheria<\/a>, <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.health.gov.au\/health-topics\/tetanus\">tetanus<\/a>, <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.health.gov.au\/health-topics\/whooping-cough-pertussis\">pertussis<\/a>) booster and <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.health.gov.au\/health-topics\/meningococcal-disease\">meningococcal ACWY<\/a> disease.<\/p>\n<p>It makes good sense to include COVID vaccines as part of the tried and trusted school-based immunization program. <\/p>\n<p><b>Who gives consent?<\/b><\/p>\n<p>Because these immunisations are given through schools, adolescent vaccination in Australia is a partnership between health and education departments.<\/p>\n<p>Written parental or guardian consent is required prior to the administration of vaccines.<\/p>\n<p>This differs to what&#8217;s done through health settings, such as <a href=\"https:\/\/buradabiliyorum.com\/en\/category\/general\/\" data-internallinksmanager029f6b8e52c=\"3\" title=\"General\" target=\"_blank\" rel=\"noopener\">general<\/a> practice, where adolescent &#8220;<a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.health.nsw.gov.au\/policies\/manuals\/Documents\/consent-section-8.pdf\">competency to consent<\/a>&#8221; is an important consideration.<\/p>\n<p>&#8220;Competency to consent&#8221; refers to the capacity of someone under 18 to consent to or refuse medical treatment. It signifies the minor has reached sufficient intelligence and understanding to fully understand the proposed treatment. The seriousness of the treatment is taken into consideration and capacity is assessed by individual health professionals. If deemed competent, then there&#8217;s no legal requirement for parental or guardian consent. Although, parental consent in addition to adolescent consent is encouraged as best practice.<\/p>\n<p><b>2 approaches to adolescent COVID vaccination<\/b><\/p>\n<p>First, we must improve adolescents&#8217; understanding of vaccination to support their involvement in decision-making. <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.tandfonline.com\/doi\/full\/10.1080\/14681811.2017.1300770\">In our own research<\/a> about HPV vaccination, we found information designed specifically for adolescents is important. Adolescents otherwise have limited understanding of the vaccines they receive, or the diseases they prevent.<\/p>\n<p>Even if consent from a parent or guardian is required as it is in the school-based program, promoting vaccine literacy among adolescents is appropriate and ethical. Understanding the purpose and process of vaccination increases vaccine confidence and reduces fear and anxiety.<\/p>\n<p>Second, we need to acknowledge adolescents&#8217; <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.racgp.org.au\/download\/documents\/AFP\/2011\/March\/201103bird.pdf\">legal right to consent<\/a> where they are competent to do so. This is pertinent where parental consent isn&#8217;t obtained, often due to a simple failure to return a consent form in time.<\/p>\n<p>Where this happens, a GP can obtain informed consent in the usual way for medical treatment.<\/p>\n<p>However, the requirement to access a GP practice presents <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/onlinelibrary.wiley.com\/doi\/pdfdirect\/10.1111\/1753-6405.12936\">other barriers for mature minors<\/a> (those under 18 years who are competent to consent), which may impede vaccine uptake.<\/p>\n<p>The national imperative is to achieve as high coverage of COVID vaccination as possible across the population. To achieve this and reduce the impact of the pandemic on young people&#8217;s health, we must work with young people. <\/p>\n<p><b>What do young people think?<\/b><\/p>\n<p>The National Health and Medical Research Council (NHMRC) has established a network of researchers who champion adolescent health, called the <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.why.org.au\/About-us\">Wellbeing Health &amp; Youth Centre<\/a>.<\/p>\n<p>This network has created the <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.why.org.au\/why-research\/ethics-and-engagement\/WHY-Commission\">WH&amp;Y Commission<\/a>, which includes the voices of young people. Its goal is to ensure adolescent health research and policymaking are guided by young people themselves.<\/p>\n<p>We asked three young WH&amp;Y Commissioners what they thought about the issue of COVID vaccines and adolescents.<\/p>\n<p><b>Here&#8217;s what they had to say<\/b><\/p>\n<p>Young people should be given unbiased, accurate information about the benefits and risks associated with COVID vaccines.<\/p>\n<p>Young people understand that because it&#8217;s a new vaccine, there will inevitably be skepticism. They&#8217;re aware family members may be hesitant or hold opposing views, which could deny young people their right to be fully informed. They want transparent instruction and information to be a huge priority for governments. <\/p>\n<p>Young people deemed competent should be afforded their legal right to consent to a COVID vaccine. This would acknowledge the autonomy of, and trust placed in, young people to make their own medical decisions. There should be appropriate structures in place to protect young people&#8217;s privacy in their decision-making process. This is important to avoid stigmatization based on their choice.<\/p>\n<p>Australian Common Law reflects the understanding that over the second decade of life, young people gain autonomy over their lives and are <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.health.nsw.gov.au\/policies\/manuals\/Documents\/consent-section-8.pdf\">capable of making decisions<\/a> about their own health care.
