\u2018exceptionally bad\u2019 mistakes kill mom hours after giving birth&#8221;<span class=\"ez-toc-section-end\"><\/span><\/h1>\n<p><img decoding=\"async\" src=\"https:\/\/nypost.com\/wp-content\/uploads\/sites\/2\/2022\/06\/dead-mother-247.jpg?quality=90&amp;strip=all\" \/><\/p>\n<div>\n<aside class=\"single__inline-module alignleft\">\n<\/aside>\n<p>Serious errors from senior staff members at a West Yorkshire Hospital led to the death of a first-time mom just hours after she gave birth, an inquest has found.\u00a0<\/p>\n<p>Jade\u00a0Hart, who was originally from Pontefract, gave birth to her son Hugo just after 10 p.m. on July 8, 2018, at Bassetlaw Hospital in Worksop.<\/p>\n<p>But hours later she tragically died, having suffered a uterine inversion \u2014 a rare complication in which the uterus turns partially or completely inside out.<\/p>\n<p>An inquest at\u00a0Nottingham\u00a0Coroner\u2019s Court has concluded this occurred as a result of mistakes carried out by one of her doctors and a midwife at the hospital.<\/p>\n<p>The 33-year-old\u2019s umbilical cord was found to have been pulled at least four times by a doctor when her placenta had not separated.<\/p>\n<p>Assistant Coroner Dr. Elizabeth Didcock labelled the action as \u201ca very serious omission of care\u201d which led to Mrs. Hart\u2019s death.<\/p>\n<p>A report issued at the Inquest found that the mother had suffered multiple cardiac arrests and would not have died if it were not for the uterine inversion.<\/p>\n<p>The investigation also found that the mismanagement of an ongoing hemorrhage also contributed to her death, with regular checks not undertaken in the hours leading up to her death.<\/p>\n<p>Reports said she had lost at least 5.5 liters of blood and eventually experienced organ failure and brain damage, with neglect contributing to her death.<\/p>\n<p>These individual mistakes could not have been foreseen by Doncaster and Bassetlaw Teaching Hospitals NHS Foundation Trust, the inquest concluded, and came instead as a result of guidance not being followed.<\/p>\n<p>The trust allegedly responsible has since been criticised by the coroner for a report passed on by them to medical bodies following Mrs. Hart\u2019s death.<\/p>\n<p>Speaking after the inquest, Julie Hazelgrave,\u00a0Jade\u2019s mother, paid tribute to her \u201cwonderfully kind and caring\u201d daughter who had \u201ca great zest for life.\u201d<\/p>\n<aside class=\"single__inline-module alignright\">\n    <\/aside>\n<p>She said: \u201cIn July 2018, with her husband,\u00a0Jade\u00a0walked into Bassetlaw hospital full of excitement and anticipation for the birth of their baby.<\/p>\n<p>\u201cJade\u00a0had a perfect life and marriage and, when their son was born, I saw that\u00a0Jade\u00a0was the h<a href=\"https:\/\/buradabiliyorum.com\/en\/category\/download-scripts-themes-apps\/\" data-internallinksmanager029f6b8e52c=\"9\" title=\"Download Scripts &amp; Themes &amp; Apps\" target=\"_blank\" rel=\"noopener\">app<\/a>iest she had ever been.<\/p>\n<p>\u201cCruelly, all happiness was short-lived when, within one hour of birth,\u00a0Jade\u00a0collapsed and died in the im<a href=\"https:\/\/buradabiliyorum.com\/en\/category\/social-mediaa\/\" data-internallinksmanager029f6b8e52c=\"1\" title=\"Social Media\" target=\"_blank\" rel=\"noopener\">media<\/a>te hours thereafter.<\/p>\n<p>\u201cWe remain devastated at the loss of\u00a0Jade. Her death has cast a long shadow over our lives and our world has irrevocably changed. We miss\u00a0Jade\u00a0every single day and always will.\u201d<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Exceptionally_bad_mistakes\"><\/span>Exceptionally bad mistakes<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>A well-respected expert, who undertook an independent review of Mrs. Hart\u2019s death and its circumstances, concluded that\u00a0Jade\u2019s doctor had breached their duty of care \u2014 and their actions went \u201cwell below\u201d what would be expected of them.<\/p>\n<p>They added that it was a \u201ctruly exceptionally bad\u201d decision that would result in a \u201cserious risk of death\u201d. The same expert also concluded the mismanagement of Mrs. Hart\u2019s hemorrhage also represented a gross breach of duty.<\/p>\n<p>Mrs. Hart was said to be previously well before the incident, however, she did have high blood pressure prior to labor being induced.<\/p>\n<aside class=\"single__inline-module alignleft\">\n    <\/aside>\n<p>Before giving birth, Mrs. Hart had asked for an epidural which had been rejected over fears around her high blood pressure \u2014 the inquest found this request should have been granted and would have offered her some pain relief.<\/p>\n<p>Dr. Didcock told the inquest: \u201cSerious omissions has led to insufficient learning and no acceptance that this had caused serious distress to the family.<\/p>\n<p>\u201cThis poses a risk of similar deaths occurring in the future. I am not reassured the necessary actions are in place.\u201d<\/p>\n<p>She told the family: \u201cI\u2019m so sorry for your loss. It\u2019s also my hope that lessons will be learned that will lead to improvements in care going forward.\u201d<\/p>\n<p>Jade\u00a0Hart\u2019s cause of death was given as brain injury, multiple cardiac arrests and uterine inversion from inappropriate management during third stage labour.<\/p>\n<p>The Doncaster and Bassetlaw Teaching Hospitals NHS Foundation Trust have been contacted for comment.\n                        <\/p><\/div>\n<blockquote><p><strong><span style=\"color: #ff6600;\">If you liked the article, do not forget to share it with your friends.
