Does it work? Is there evidence of harm?<\/p>\n<p>We&#8217;re a team of researchers who specialize in evidence-based musculoskeletal health. I (Matt) am a registered chiropractor, Joshua is a registered physiotherapist and Giovanni trained as a physiotherapist.<\/p>\n<p>Here&#8217;s what the evidence says.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Remind_me_how_did_this_all_come_about\"><\/span>Remind me, how did this all come about?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>A Melbourne-based chiropractor <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.9news.com.au\/national\/melbourne-news-chiropractor-under-fire-for-baby-treatment-video\/221b8793-2fc2-4414-a58d-bd6ded68996f\">posted a video<\/a> on <a href=\"https:\/\/buradabiliyorum.com\/en\/category\/social-mediaa\/\" data-internallinksmanager029f6b8e52c=\"1\" title=\"Social Media\" target=\"_blank\" rel=\"noopener\">social media<\/a> in 2018 using a spring-loaded device (known as the Activator) to manipulate the spine of a two-week-old baby suspended upside down by the ankles.<\/p>\n<p>The video sparked widespread concerns among the public, medical associations and politicians. It prompted a ban on the procedure in young children. The Victorian health minister commissioned Safer Care Victoria to conduct an <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.safercare.vic.gov.au\/best-practice-improvement\/publications\/chiropractic-spinal-manipulation-of-children-under-12\">independent review<\/a> of spinal manipulation techniques on children.<\/p>\n<p>Recently, the Chiropractic Board of Australia reinstated chiropractors&#8217; authorization to perform spinal manipulation on babies under two years old. But this week, it <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.chiropracticboard.gov.au\/News\/2024-06-17-Chiropractic-Board-reinstates-interim-policy.aspx\">backflipped<\/a>, following heavy criticism from medical associations and politicians.<\/p>\n<p>                                                                                                        <!-- Google middle Adsense block --><\/p>\n<h2><span class=\"ez-toc-section\" id=\"What_is_spinal_manipulation\"><\/span>What is spinal manipulation?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Spinal manipulation is a treatment used by chiropractors and other health professionals such as doctors, osteopaths and physiotherapists.<\/p>\n<p>It is an umbrella term that includes popular &#8220;<a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.smh.com.au\/lifestyle\/health-and-wellness\/why-does-back-cracking-feel-so-good-the-pop-explained-20201210-p56mhu.html\">back cracking<\/a>&#8221; techniques.<\/p>\n<p>It also includes more gentle forms of treatment, such as massage or joint mobilizations. These involve <a href=\"https:\/\/buradabiliyorum.com\/en\/category\/download-scripts-themes-apps\/\" data-internallinksmanager029f6b8e52c=\"9\" title=\"Download Scripts &amp; Themes &amp; Apps\" target=\"_blank\" rel=\"noopener\">app<\/a>lying pressure to joints without generating a &#8220;cracking&#8221; sound.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Does_spinal_manipulation_in_babies_work\"><\/span>Does spinal manipulation in babies work?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Several <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.nice.org.uk\/guidance\/ng59\/chapter\/Recommendations\">international<\/a> <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.acpjournals.org\/doi\/10.7326\/M16-2367\">guidelines<\/a> for health-care professionals recommend spinal manipulation to treat adults with conditions such as back pain and headache as there is an abundance of evidence on the topic. For example, spinal manipulation for back pain is <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.bmj.com\/content\/364\/bmj.l689\">supported by data<\/a> from nearly 10,000 adults.<\/p>\n<p>For children, it&#8217;s a different story. Safer Care Victoria&#8217;s 2019 <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.safercare.vic.gov.au\/sites\/default\/files\/2019-10\/20191024-Final%20Chiropractic%20Spinal%20Manipulation.pdf\">review<\/a> of spinal manipulation found very few studies testing whether this treatment was safe and effective in children.<\/p>\n<p>Studies were <a href=\"https:\/\/buradabiliyorum.com\/en\/category\/general\/\" data-internallinksmanager029f6b8e52c=\"3\" title=\"General\" target=\"_blank\" rel=\"noopener\">general<\/a>ly small and were of poor quality. Some of those small, poor-quality studies, suggest spinal manipulation provides a very small benefit for back pain, colic and potentially bedwetting \u2014some common reasons for parents to take their child to see a chiropractor. But overall, the review found the overall body of evidence was very poor.<\/p>\n<p>However, for most other children&#8217;s conditions chiropractors treat\u2014such as headache, asthma, otitis media (a type of ear infection), cerebral palsy, hyperactivity and torticollis (&#8220;twisted neck&#8221;)\u2014there did not appear to be a benefit.<\/p>\n<p>The number of studies investigating the effectiveness of spinal manipulation on babies under two years of age was even smaller.<\/p>\n<p>There was one high-quality study and two small, poor quality studies. These did not show an appreciable benefit of spinal manipulation on <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/link.springer.com\/article\/10.1186\/s12998-021-00371-8\">colic<\/a>, <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S0161475499700618\">otitis media with effusion<\/a> (known as glue ear) or <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/onlinelibrary.wiley.com\/doi\/abs\/10.1111\/j.1651-2227.2011.02145.x\">twisted neck<\/a> in babies.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Is_spinal_manipulation_on_babies_safe\"><\/span>Is spinal manipulation on babies safe?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>In terms of safety, most studies in the review found serious complications were extremely rare. The review noted one baby or child dying (a report from Germany in 2001 after spinal manipulation by a physiotherapist). The most common complications were mild in nature such as increased crying and soreness.<\/p>\n<p>However, because studies were very small, they cannot tell us anything about the safety of spinal manipulation in a reliable way. Studies that are designed to properly investigate if a treatment is safe typically include thousands of patients. And these studies have not yet been done.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Why_do_people_see_chiropractors\"><\/span>Why do people see chiropractors?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Safer Care Victoria also conducted <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/www.safercare.vic.gov.au\/sites\/default\/files\/2019-10\/20191024-Final%20Chiropractic%20Spinal%20Manipulation.pdf\">surveys<\/a> with more than 20,000 people living in Australia who had taken their children under 12 years old to a chiropractor in the past ten years.<\/p>\n<p>Nearly three-quarters said that was for treatment of a child aged two years or younger.<\/p>\n<p>Nearly all people surveyed reported a positive experience when they took their child to a chiropractor and reported that their child&#8217;s condition improved with chiropractic care.
