Over the past few years, interest in using AI to design drugs and optimise clinical trials has driven a surge in research and investment. AI-driven platforms like <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/doi.org\/10.1038\/d41586-024-03214-7\" target=\"_blank\" rel=\"nofollow noopener\">AlphaFold<\/a>, which won the <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/theconversation.com\/machine-learning-cracked-the-protein-folding-problem-and-won-the-2024-nobel-prize-in-chemistry-240937\" target=\"_blank\" rel=\"nofollow noopener\">2024 Nobel Prize<\/a> for its ability to predict the structure of proteins and design new ones, showcase AI\u2019s potential to accelerate drug development.<\/p>\n<p>AI in drug discovery <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/www.statnews.com\/2024\/11\/20\/schrodinger-ramy-farid-drug-discovery-ai-physics\/\" target=\"_blank\" rel=\"nofollow noopener\">is \u201cnonsense<\/a>,\u201d warn some industry veterans. They urge that \u201cAI\u2019s potential to accelerate drug discovery needs a <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/doi.org\/10.1038\/d41586-023-03172-6\" target=\"_blank\" rel=\"nofollow noopener\">reality check<\/a>,\u201d as AI-generated drugs have yet to demonstrate an ability to address the <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/theconversation.com\/90-of-drugs-fail-clinical-trials-heres-one-way-researchers-can-select-better-drug-candidates-174152\" target=\"_blank\" rel=\"nofollow noopener\">90% failure rate<\/a> of new drugs in clinical trials. We argue that AI in drug development is not yet a <a href=\"https:\/\/buradabiliyorum.com\/en\/category\/game\/\" data-internallinksmanager029f6b8e52c=\"7\" title=\"Game\" target=\"_blank\" rel=\"noopener\">game<\/a>-changer, nor is it complete nonsense. AI is not a black box that can turn any idea into gold. Rather, we see it as a tool that, when used wisely and competently, could help address the root causes of drug failure and streamline the process.<\/p>\n<p>Most work using AI in drug development intends to reduce the <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/theconversation.com\/90-of-drugs-fail-clinical-trials-heres-one-way-researchers-can-select-better-drug-candidates-174152\" target=\"_blank\" rel=\"nofollow noopener\">time and money<\/a> it takes to bring one drug to market \u2013 currently 10 to 15 years and US$1 billion to $2 billion. But can AI truly revolutionise drug development and improve success rates?<\/p>\n<h2><span class=\"ez-toc-section\" id=\"AI_in_drug_development\"><\/span>AI in drug development<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Researchers have <a href=\"https:\/\/buradabiliyorum.com\/en\/category\/download-scripts-themes-apps\/\" data-internallinksmanager029f6b8e52c=\"9\" title=\"Download Scripts &amp; Themes &amp; Apps\" target=\"_blank\" rel=\"noopener\">app<\/a>lied AI and machine learning to <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/doi.org\/10.1021\/acs.jmedchem.4c01684\" target=\"_blank\" rel=\"nofollow noopener\">every stage<\/a> of the drug development process. This includes identifying targets in the body, screening potential candidates, designing drug molecules, predicting toxicity and selecting patients who might respond best to the drugs in clinical trials, among others.<\/p>\n<p>Between 2010 and 2022, <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/doi.org\/10.1038\/d41573-022-00025-1\" target=\"_blank\" rel=\"nofollow noopener\">20 AI-focused startups<\/a> discovered 158 drug candidates, 15 of which advanced to clinical trials. Some of these drug candidates were able to complete preclinical testing in the lab and enter human trials in just 30 months, compared with the typical <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/doi.org\/10.1016\/j.apsb.2022.02.002\" target=\"_blank\" rel=\"nofollow noopener\">3 to 6 years<\/a>. This accomplishment demonstrates AI\u2019s potential to accelerate drug development.<\/p>\n<figure>\n<div class=\"resp-video-container\"><iframe srcdoc=\"&lt;style&gt;*{padding:0;margin:0;overflow:hidden}html,body{background:#000;height:100%}img{position:absolute;top:0;left:0;width:100%;height:100%;object-fit:cover;transition:opacity .1s cubic-bezier(0.4,0,1,1)}a:hover img+img{opacity:1!important}&lt;\/style&gt;&lt;a href=\" https:=\"\"><img decoding=\"async\" src=\"https:\/\/img.youtube.com\/vi\/jZzB9qrPFzg\/hqdefault.jpg\"\/><img decoding=\"async\" src=\"https:\/\/cdn0.tnwcdn.com\/wp-content\/themes\/cyberdelia\/assets\/img\/ytplaybtn.png\" style=\"top: 50%;left:50%;width:68px;height:48px;transform:translate3d(-50%,-50%,0)\"\/><img decoding=\"async\" src=\"https:\/\/cdn0.tnwcdn.com\/wp-content\/themes\/cyberdelia\/assets\/img\/ytplaybtn-hover.png\" style=\"top: 50%;left:50%;width:68px;height:48px;opacity:0;transform:translate3d(-50%,-50%,0)\"\/>&#8221; width=&#8221;440&#8243; height=&#8221;260&#8243; frameborder=&#8221;0&#8243; allowfullscreen=&#8221;allowfullscreen&#8221;&gt;<\/iframe><\/div><figcaption><span class=\"caption\">Drug development is a long and costly process.<\/span><\/figcaption><\/figure>\n<p>On the other hand, while AI platforms may rapidly identify compounds that work on cells in a Petri dish or in animal models, the success of these candidates in clinical trials \u2013 where the majority of drug failures occur \u2013 remains <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/www.statnews.com\/2024\/12\/03\/ai-drug-discovery-investors-insilico-recursion\/\" target=\"_blank\" rel=\"nofollow noopener\">highly uncertain<\/a>.<\/p>\n<p>Unlike other fields that have large, high-quality datasets available to train AI models, such as image analysis and language processing, the AI in drug development is constrained by <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/doi.org\/10.1038\/d41586-023-02896-9\" target=\"_blank\" rel=\"nofollow noopener\">small, low-quality datasets<\/a>. It is difficult to generate drug-related datasets on cells, animals or humans for millions to billions of compounds. While <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/doi.org\/10.1038\/s41586-024-07487-w\" target=\"_blank\" rel=\"nofollow noopener\">AlphaFold<\/a> is a breakthrough in predicting protein structures, <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/doi.org\/10.1038\/d41586-024-01383-z\" target=\"_blank\" rel=\"nofollow noopener\">how precise<\/a> it can be for drug design remains uncertain. Minor changes to a drug\u2019s structure can greatly affect its activity in the body and thus how effective it is in treating disease.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Survivorship_bias\"><\/span>Survivorship bias<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Like AI, past innovations in drug development like <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/doi.org\/10.1038\/s41586-023-05905-z\" target=\"_blank\" rel=\"nofollow noopener\">computer-aided drug design<\/a>, the <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/doi.org\/10.1038\/d41586-021-00314-6\" target=\"_blank\" rel=\"nofollow noopener\">Human Genome Project<\/a> and <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/theconversation.com\/discovering-new-drugs-is-a-long-and-expensive-process-chemical-compounds-that-dupe-screening-tools-make-it-even-harder-175972\" target=\"_blank\" rel=\"nofollow noopener\">high-throughput screening<\/a> have improved individual steps of the process in the past 40 years, yet drug failure rates <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/theconversation.com\/90-of-drugs-fail-clinical-trials-heres-one-way-researchers-can-select-better-drug-candidates-174152\" target=\"_blank\" rel=\"nofollow noopener\">haven\u2019t improved<\/a>.<\/p>\n<p>Most AI researchers can tackle specific tasks in the drug development process when provided with high-quality data and particular questions to answer. But they are often <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/doi.org\/10.1021\/acs.jmedchem.4c01684\" target=\"_blank\" rel=\"nofollow noopener\">unfamiliar with the full scope<\/a> of drug development, reducing challenges into pattern recognition problems and refinement of individual steps of the process. Meanwhile, many scientists with expertise in drug development lack training in AI and machine learning. These communication barriers can hinder scientists from moving beyond the mechanics of current development processes and identifying the root causes of drug failures.<\/p>\n<p>Current approaches to drug development, including those using AI, may have fallen into a <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/www.britannica.com\/science\/survivorship-bias\" target=\"_blank\" rel=\"nofollow noopener\">survivorship bias<\/a> trap, overly focusing on less critical aspects of the process while <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/doi.org\/10.1021\/acs.jmedchem.4c01684\" target=\"_blank\" rel=\"nofollow noopener\">overlooking major problems<\/a> that contribute most to failure. This is analogous to repairing damage to the wings of aircraft returning from the battle fields in World War II while neglecting the fatal vulnerabilities in engines or cockpits of the planes that never made it back. While AI may be able to reduce the time and cost of the lab-based preclinical stages of this assembly line, it is unlikely to boost success rates in the more costly clinical stages that involve testing in people. The persistent <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/theconversation.com\/90-of-drugs-fail-clinical-trials-heres-one-way-researchers-can-select-better-drug-candidates-174152\" target=\"_blank\" rel=\"nofollow noopener\">90% failure rate<\/a> of drugs in clinical trials, despite 40 years of process improvements, underscores this limitation.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Addressing_root_causes\"><\/span>Addressing root causes<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Drug failures in clinical trials are not solely due to how these studies are designed; selecting the <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/doi.org\/10.1021\/acs.jmedchem.4c01684\" target=\"_blank\" rel=\"nofollow noopener\">wrong drug candidates<\/a> to test in clinical trials is also a major factor. New AI-guided strategies could help address both of these challenges.<\/p>\n<p>Currently, <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/doi.org\/10.1038\/nrd.2016.184\" target=\"_blank\" rel=\"nofollow noopener\">three interdependent factors<\/a> drive most drug failures: dosage, safety and efficacy. Some drugs fail because they\u2019re too toxic, or unsafe. Other drugs fail because they\u2019re deemed ineffective, often because the dose can\u2019t be increased any further without causing harm.<\/p>\n<p>We and our colleagues propose a <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/doi.org\/10.1021\/acs.jmedchem.4c01684\" target=\"_blank\" rel=\"nofollow noopener\">machine learning system<\/a> to help select drug candidates by predicting dosage, <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/doi.org\/10.1007\/s11095-024-03742-x\" target=\"_blank\" rel=\"nofollow noopener\">safety<\/a> and efficacy based on five previously overlooked features of drugs. Specifically, researchers could use AI models to determine how specifically and potently the drug binds to known and unknown targets, the level of these targets in the body, how concentrated the drug becomes in healthy and diseased tissues, and the drug\u2019s structural properties.<\/p>\n<p>These features of AI-generated drugs could be tested in what we call <a rel=\"nofollow\" target=\"_blank\" href=\"https:\/\/doi.org\/10.1021\/acs.jmedchem.4c01684\" target=\"_blank\" rel=\"nofollow noopener\">phase 0+ trials<\/a>, using ultra-low doses in patients with severe and mild disease. This could help researchers identify optimal drugs while reducing the costs of the current \u201ctest-and-see\u201d approach to clinical trials.<\/p>\n<p>While AI alone might not revolutionise drug development, it can help address the root causes of why drugs fail and streamline the lengthy process to approval.<!-- Below is The Conversation's page counter tag.
