Nimaroff, MD, senior vice president and executive director of obstetrics and gynecology for <a rel=\"nofollow noopener noreferrer\" target=\"_blank\" href=\"https:\/\/www.northwell.edu\/\">Northwell Health<\/a> and chairman of the department of obstetrics and gynecology at <a rel=\"nofollow noopener noreferrer\" target=\"_blank\" href=\"https:\/\/nsuh.northwell.edu\/\">North Shore University Hospital<\/a> and <a rel=\"nofollow noopener noreferrer\" target=\"_blank\" href=\"https:\/\/lij.northwell.edu\/\">Long Island Jewish Medical Center.<\/a><\/p>\n<p>Fortunately, with treatment, most patients with this issue can have a successful outcome. Treatment includes administration of progesterone, and placement of a vaginal cerclage \u2014 a suture placed in the cervix by 12 weeks of pregnancy \u2014 which is over 65 percent successful.<\/p>\n<p>Unfortunately, for Wehye, the fix came too late, and didn\u2019t last.<\/p>\n<p>\u201cI hardly had any cervix at that point, from what I gather,\u201d she said. \u201cBy week\u2019s end, my water broke and the doctors said they needed to deliver the baby, who was 22 weeks.\u201d<\/p>\n<p>Although initially against it, \u201cthey told me I\u2019d die of sepsis, and the baby wouldn\u2019t make it either. It was the worst day, the hardest thing, but her lungs were not strong enough,\u201d said Wehye, who was ultimately induced into labor.<\/p>\n<p>\u201cI delivered my daughter, Chloe. She was such a strong little girl. She came out alive and breathing. I was able to talk to her and kiss her,\u201d said Wehye. \u201cMy family was able to give her love for an hour and a half, and then she died.\u201d<\/p>\n<p>Wehye\u2019s relationship with Chloe\u2019s father also ended, and though distraught for \u201ca very long time \u2014 years,\u201d Wehye later discussed having a child with her fianc\u00e9, Marc Nightingale, 49. But after her tragic loss, Wehye was scared to get pregnant.<\/p>\n<p>\u201cBut I\u2019d had no prior history [of a problem]. My earlier doctors didn\u2019t check further. If they had caught this early enough, they could have tried a cerclage earlier on in my pregnancy. I didn\u2019t know this. I was told what h<a href=\"https:\/\/buradabiliyorum.com\/en\/category\/download-scripts-themes-apps\/\" data-internallinksmanager029f6b8e52c=\"9\" title=\"Download Scripts &amp; Themes &amp; Apps\" target=\"_blank\" rel=\"noopener\">app<\/a>ened to me was a fluke,\u201d she said.<\/p>\n<p>Disturbed and dissatisfied, Wehye researched her condition.<\/p>\n<p>\u201cI wanted to know what my cervix was like. Apparently, it\u2019s in the normal range when not pregnant, but once I\u2019m in the second trimester, and the baby gets some weight on, my cervix can\u2019t handle it,\u201d she said.<\/p>\n<p>Wehye learned of a process called transabdominal cerclage (TAC), and found Dr. Nimaroff in New York, one of only a handful of doctors in the area who perform the surgery. \u201cIn patients who have an extremely short cervix or who have failed a prior vaginal cerclage, placement of the cerclage abdominally can offer hope,\u201d said Dr. Nimaroff.<\/p>\n<p>\u201cOften, this surgery can be accomplished through a minimally invasive approach,\u201d he said. \u201cIt has an over 80 percent success rate. I\u2019ve been doing these procedures for 10 years \u2014 they\u2019re complicated surgeries but are very effective for those with truly challenging histories, who are so anxious to have a family and who have given up hope. It can be a life-changing procedure.\u201d<\/p>\n<p>In March 2019, Wehye met with Dr. Nimaroff.<\/p>\n<p>\u201cHe said, \u2018We\u2019re going to fix this. We\u2019re going to put this to bed.\u2019 I welled up with tears. I\u2019d needed to hear that from a doctor who would go to war with me,\u201d said Wehye.<\/p>\n<p>In May 2019, Wehye underwent a successful TAC surgery. She was pregnant just a couple of weeks later.<\/p>\n<p>The TAC did what it was supposed to do and at 37 weeks and a day, baby Violet Amaya was born via cesarean section in February.<\/p>\n<p>\u201cI bawled my eyes out. Without [Dr. Nimaroff] and the work he does, this wouldn\u2019t have happened,\u201d said Wehye.<\/p>\n<p>The other good <a href=\"https:\/\/buradabiliyorum.com\/en\/category\/news\/\" data-internallinksmanager029f6b8e52c=\"2\" title=\"News\" target=\"_blank\" rel=\"noopener\">news<\/a> is that the stitch can be left in place for future pregnancies. \u201cI\u2019ve had patients who have had multiple pregnancies with the same cerclage,\u201d said Dr. Nimaroff.<\/p>\n<p>For those in similar situations, \u201cYou have to be your own advocate,\u201d said Wehye. \u201cTrust your gut. I had a lot of guilt after my first loss. I thought, \u2018Why didn\u2019t I ask questions?\u2019 You need a doctor who will be on your team and explain things to you in a way that you understand,\u201d said Wehye. \u201cIf it wasn\u2019t for him and the care I received, I wouldn\u2019t have my daughter.\u201d<\/p>\n<h2><span class=\"ez-toc-section\" id=\"WHY_THINGS_GO_WRONG\"><\/span>WHY THINGS GO WRONG<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>A single miscarriage is extremely common. It\u2019s estimated that up to two-thirds of women will experience a loss in the first trimester.<\/p>\n<p>\u201cIn some cases, [you] might not even be aware of the loss if it presents as a late or missed period,\u201d said Dr. Daniel E. Stein, director of reproductive endocrinology and infertility at <a rel=\"nofollow noopener noreferrer\" target=\"_blank\" href=\"https:\/\/www.mountsinai.org\/locations\/west\">Mount Sinai West<\/a> hospital and partner at <a rel=\"nofollow noopener noreferrer\" target=\"_blank\" href=\"https:\/\/www.rmany.com\/\">Reproductive Medicine Associates of New York<\/a>.<\/p>\n<p>Recurrent losses (two or more) are far less common, probably occurring in less than 5 percent of pregnancies.<\/p>\n<p>\u201cAt least half of miscarriages are due to chromosome abnormalities, but may also include other factors, including anatomical defects in the uterus, autoimmune factors for the mom, hormonal abnormalities, blood-clotting disorders and environmental factors,\u201d said Dr. Stein. In the other half, no cause is ever found. \u201cThe next pregnancy might be completely normal, without any type of intervention,\u201d he said.<\/p>\n<p>The first course of action is a detailed look at a patient\u2019s medical history, followed by a physical exam, pelvic ultrasound, and lab tests.<\/p>\n<p>\u201cUterine, autoimmune, hormonal and clotting abnormalities can all be treated. In chromosomal cases, we can often achieve excellent success through in vitro fertilization (IVF),\u201d said Dr. Stein.<\/p>\n<p>If you\u2019ve experienced two or more losses, \u201cSee an experienced board-certified reproductive endocrinologist,\u201d said Dr. Stein. \u201cThere may very well be interventions that are highly successful.
